E-Forms

Welcome to Alfred University's E-Form repository. To fill out one of the forms listed below and then upload, do the following:

• To use this application you must have Adobe Acrobat Reader installed on your computer. If you need to install this download here.
• Right click on the form needed, click save link as or save target as (depends on your browser)
• Popup will appear to save, select where you want to save
• Click the save button
• Save the file on your computer, keeping the default filename (ie. 0014.pdf)
• Go to where you saved the file
• Double click on the file
• Fill out the form
• Instructions on including a digital signature
• Save the file again on your computer, using the same filename
• Go back to the E-Forms page at my.alfred.edu/eforms to upload the form
• Where it says upload completed forms here, click on browse
• Browse to the file on your computer, click submit

This will upload the form so the department can access it.
